<p>By examining the pathophysiology of Australia’s most common chronic and complex health conditions, students will translate evidence-based nursing care with a focus on rural, regional and remote settings. Students will investigate assessments, intervention and evaluation methods for the care of people experiencing common chronic and mental health conditions across the lifespan and explore the impact of digital health technologies. In partnership with First Nations peoples, students will examine strategies for delivering culturally safe mental health care in communities. Students will critically analyse the psycho-social, cultural, legal and ethical considerations for people across the lifespan living with chronic disease and mental health conditions, their families and carers. Students will also critically evaluate health service navigation and contemporary frameworks and models for interprofessional chronic and mental health care.  Through the appraisal of planetary, global, public, population and local health concepts, students will consider how these link to the determinants of health and their impact on chronic and mental health conditions. </p>

Health Priorities: Chronic and Complex Interprofessional Care

Critically analyse the psycho-social, cultural, legal and ethical considerations for people across the lifespan living with chronic disease and mental health conditions, including First Nations peoples.

Examine the pathophysiology of common chronic conditions to inform evidence-based person-centred nursing and interprofessional care.

Critically evaluate the health care system and service navigation for people with chronic, complex and mental health conditions, living in rural, remote and regional settings and the impact of digital health strategies.

Examine strategies for delivering culturally safe chronic and mental health care in partnership with First Nations health professionals, organisations and communities, that reflects individual, cultural and linguistic diversity and addresses potential challenges in the delivery of responsive health care practice.

Appraise planetary, global, public, population and local health concepts and their impact on the determinants of health and chronic and mental health conditions.

Interpret assessments and interventions for common chronic and mental health conditions   and the role of the interprofessional team in the management of the person living with a chronic or mental health condition for people across the lifespan.
